In Delphi 5 (and up to Delphi 7 AFAIK), you can use the Help,
Customize... menu option to actually compile/link it into your Delphi
Help.
You'll need to know the location of the .hlp file that you want to
compile in, which you do, so I would suggest that's preferable to having
it on the Tools menu.
Not sure how to do it in D2006, but if, as an example, I search for
ShellExecute in D2006, it's already there anyway on my installation.
Not sure if I did anything unusual to get it to display there.

Aha now I know where the help file is, this is another way to add it to
the Tools Menu:
In the Tools Menu, Configure Tools and Add,
Title: Delphi5 Win32SDK
Program: WINHLP32
Working Dir: C:\Program Files\Common Files\Borland Shared\MSHelp
Parameter: WIN32.HLP
